I'm not convinced about this form. It seems to me that much of what is on here is necessary for Helen or head tutor (sorry helen I'm not sure of your title) to match with the tutor. Having filled one out myself with Mark who very ably tutored me on blogging, much of the info was quite irrelevant or unnecessary. Personally I would prefer to engage with my peer and find out the info that is needed that way

From a nursing perspective, I think it needs to reflect if the student has any health needs that might impact on their learning, e.g visual disability etc.
